In open rhinoplasty, an incision is made across the columella (the strip of tissue between the nostrils), allowing the surgeon to lift the nasal skin and access the underlying structures. In closed rhinoplasty, incisions are made inside the nostrils. The surgeon will then reshape the nasal tip and bridge by removing, adding, or rearranging the cartilage and bone.
